Mosunetuzumab + polatuzumab vedotin is a fixed-duration combination regimen for the treatment of relapsed or refractory large B-cell lymphoma (LBCL), particularly in transplant-ineligible patients[1][3][5]. **Mosunetuzumab** is a bispecific CD20xCD3 T-cell-engaging monoclonal antibody that redirects endogenous T cells to eliminate malignant B cells by simultaneously binding to CD20 (on B cells) and CD3 (on T cells), activating T cell-mediated cytotoxicity[3][4]. **Polatuzumab vedotin** is an antibody-drug conjugate consisting of an anti-CD79b monoclonal antibody conjugated by a protease-cleavable linker to monomethyl auristatin E (MMAE), a cytotoxic microtubule inhibitor; after binding to CD79b, it is internalized, and MMAE is released, causing mitotic arrest and apoptosis[2][3][4]. In phase 3 SUNMO and phase 1b/2 studies, this combination provided superior progression-free survival and response rates relative to standard chemotherapies, with a manageable safety profile that enables outpatient administration. This non-chemotherapeutic, immunologic and targeted therapy regimen is viewed as a significant advance for high-risk or frail LBCL patients[1][3][5]. The combination is developed and manufactured by Roche/Genentech[1].
