Motsugibatorep (also known as motugivatrep) is a first-in-class, selective transient receptor potential vanilloid 1 (TRPV1) antagonist developed as a topical ophthalmic suspension for the treatment of dry eye disease (DED). Originally discovered by Mochida Pharmaceutical and subsequently developed and commercialized by Senju Pharmaceutical, the drug targets the TRPV1 ion channel on corneal sensory neurons and ocular surface tissues. By blocking TRPV1 activation, motsugibatorep inhibits calcium influx, suppressing neuronal depolarization, neuropeptide release, and downstream neurogenic inflammation. This mechanism reduces corneal nerve hypersensitivity and alleviates the subjective symptoms of dry eye, such as pain, photophobia, and discomfort, while also improving objective ocular surface signs. It was approved in Japan in December 2025 under the brand name Avarept.
