The MOXI antioxidant formulation is a multi-ingredient dietary supplement developed by Yale University and the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) Reproductive Medicine Network. It was specifically designed for the Males, Antioxidants, and Infertility (MOXI) trial to address male factor infertility. The formulation consists of seven active ingredients: 500 mg of vitamin C, 400 mg of vitamin E, 0.20 mg of selenium, 1,000 mg of l-carnitine, 20 mg of zinc, 1,000 μg of folic acid, and 10 mg of lycopene. Its intended mechanism of action is to reduce oxidative stress and sperm DNA fragmentation by scavenging reactive oxygen species (ROS), thereby improving semen parameters and increasing live birth rates. However, a Phase 2 multicenter trial conducted between 2015 and 2018 concluded that the supplement did not significantly improve sperm concentration, motility, DNA integrity, or pregnancy outcomes compared to placebo.
