Moxifloxacin + povidone iodine is a topical ophthalmic combination used primarily for the prophylaxis of ocular infections, especially in the context of preoperative or peri-intravitreal injection care. Moxifloxacin is a fourth-generation fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, leading to inhibition of bacterial replication and cell death. Povidone iodine is a broad-spectrum antiseptic that releases free iodine, which rapidly penetrates microbial cells and disrupts protein and nucleic acid structure, resulting in cell death. The combination aims to reduce conjunctival bacterial flora more effectively than either agent alone; some studies suggest synergistic effects in reducing ocular surface bacteria[3][6]. However, clinical trials have shown mixed results regarding whether adding moxifloxacin to povidone-iodine provides additional benefit over povidone-iodine alone for preoperative prophylaxis[1][2].
