Overview

MPC-4326 + optimized background regimen is a combination therapy for HIV-1 infection consisting of the investigational drug MPC-4326 (also known as bevirimat or bevirimat dimeglumine) administered together with an individualized, patient-specific "optimized background regimen" (OBR) of 2–3 antiretroviral drugs. MPC-4326 is a first-in-class small molecule inhibitor of HIV-1 maturation that acts by interfering with the final step in processing the HIV-1 Gag protein, resulting in noninfectious, immature virus particles and preventing further rounds of infection. The OBR component consists of other antiretroviral agents selected based on resistance profiles and prior treatment history to maximize efficacy for patients failing current regimens due to multidrug resistance. This combination was studied primarily in treatment-experienced individuals with documented resistance to at least one agent from each major ARV class (NRTI, NNRTI, PI). Clinical trials demonstrated that this approach could achieve clinically relevant reductions in viral load for a subset (~60–70%) of patients identified by specific viral genotypes[5][7][10].
