A combination cell therapy in which one unit of human umbilical cord blood is expanded ex vivo by co-culturing with **mesenchymal precursor cells (MPCs)** and then infused together with an additional unmanipulated (unexpanded) cord blood unit. This regimen is used primarily as part of hematopoietic cell transplantation for patients with hematologic malignancies, especially following reduced-intensity conditioning or myeloablative regimens[1]. The expansion process increases the total nucleated cell (TNC) and CD34+ hematopoietic progenitor cell dose, resulting in improved time to neutrophil and platelet engraftment compared to transplantation with two unmanipulated cord blood units[1][2]. The therapy leverages both the enhanced cell dose from ex vivo expansion and the functional diversity of unmanipulated cord blood to optimize engraftment and recovery. Primary indications include transplantation in patients with leukemia, lymphoma, and other hematological cancers. The developer of this particular approach includes the University of Texas MD Anderson Cancer Center.
