mPENTOCLO (modified PENTOCLO) is an oral combination drug regimen consisting of three active ingredients: pentoxifylline, tocopherol (Vitamin E), and clodronate. Developed by Sunnybrook Health Sciences Centre, it is primarily investigated for the treatment of osteoradionecrosis (ORN) of the mandible, a severe complication of head and neck radiotherapy characterized by necrotic bone exposure. The regimen is designed to reverse radiation-induced fibrosis (RIF) and promote bone healing through a multi-targeted approach. Pentoxifylline acts as a hemorrheologic agent and non-selective phosphodiesterase inhibitor that improves microcirculation and reduces fibrosis by inhibiting TNF-alpha; tocopherol serves as an antioxidant to neutralize reactive oxygen species and mitigate oxidative damage; and clodronate, a non-nitrogenous bisphosphonate, inhibits osteoclast-mediated bone resorption and may stimulate osteogenesis. The 'm' prefix denotes a modified version of the original PENTOCLO protocol, which typically includes a pre-treatment phase with antibiotics followed by long-term administration of the core combination for up to 12 months.
