MPS cardioplegic formula refers to a microplegia solution typically delivered using the Quest Medical Myocardial Protection System (MPS) during cardiac surgery. Unlike traditional large-volume crystalloid cardioplegia, this formula utilizes the patient's own autologous blood as the primary vehicle, which is precisely titrated with small volumes of concentrated additives including potassium chloride, magnesium sulfate, and lidocaine. The primary mechanism of action involves inducing rapid diastolic cardiac arrest through hyperkalemic depolarization of the myocardial cell membrane, while magnesium and lidocaine provide additional membrane stabilization and calcium antagonism to minimize ischemic injury. This approach is designed to reduce hemodilution, improve oxygen delivery to the myocardium during arrest, and allow for real-time adjustment of the cardioplegia composition based on the patient's physiological needs during cardiopulmonary bypass.
