MR-L45 is a first-in-class small molecule allosteric activator of the phosphodiesterase 4 (PDE4) enzyme, developed by Mironid Limited for the treatment of autosomal dominant polycystic kidney disease (ADPKD). Unlike traditional PDE4 inhibitors that increase cyclic adenosine monophosphate (cAMP) levels to treat inflammatory conditions, MR-L45 utilizes Mironid's proprietary LoAc® (Local Activation) technology to enhance the enzymatic activity of PDE4, thereby promoting the degradation of cAMP. In ADPKD, elevated cAMP levels in renal tubular cells drive cyst proliferation and fluid secretion; by lowering cAMP concentrations, MR-L45 aims to suppress cyst growth and preserve kidney function. The drug is designed for oral administration and represents a novel therapeutic strategy for managing the progression of polycystic kidney disease.
