MRK-409 (also known as MK-0343) is a small molecule, subtype-selective partial agonist of the GABAA receptor that was developed by Merck & Co. for the treatment of anxiety disorders. It acts primarily at the benzodiazepine binding site of GABAA receptors, with high affinity for α1-, α2-, α3-, and α5-containing subtypes. Preclinically, MRK-409 demonstrated anxiolytic-like activity in rodent and primate models without causing significant sedation at effective doses. However, in human studies, it caused pronounced sedation even at low levels of receptor occupancy (<10%), which led to discontinuation of its development for anxiety disorders[1][5][8]. The drug's mechanism involves positive allosteric modulation (partial agonism) at specific GABAA receptor subunits.
