Overview

The MRKAd5 HIV-1 gag vaccine is an investigational therapeutic and preventive DNA vaccine developed for the prevention of human immunodeficiency virus type 1 (HIV-1) infection. It uses a replication-incompetent recombinant adenovirus serotype 5 (Ad5) vector to deliver the near-consensus clade B *gag* gene from HIV-1 into host cells, aiming to elicit strong cellular immune responses against the virus[1][6]. The primary mechanism involves stimulating cytotoxic T lymphocyte (CTL) responses specific to the Gag protein of HIV-1. Developed by Merck, this candidate was evaluated in multiple international clinical trials for safety and immunogenicity. While it induced robust immune responses in healthy volunteers across diverse geographic regions, it ultimately failed to demonstrate efficacy in preventing HIV acquisition or reducing viral load in large-scale efficacy trials[2][4]. The most common adverse events were mild-to-moderate injection site reactions and flu-like symptoms[1].
