MSC-Delta-24-RGD is an investigational cell-based oncolytic viral therapy designed to treat high-grade gliomas. It consists of human bone marrow-derived mesenchymal stem cells (hMSCs) loaded with Delta-24-RGD (also known as DNX-2401), a replication-competent oncolytic adenovirus. The virus is engineered with a 24-base pair deletion in the E1A gene, which restricts its replication to tumor cells with a deficient retinoblastoma (RB) pathway, and an RGD-4C motif in the fiber protein to enhance entry into tumor cells via integrins. The MSC carrier acts as a delivery vehicle, exploiting the natural tumor-homing properties of stem cells to enhance the intravascular delivery of the virus to the tumor microenvironment while protecting it from neutralizing antibodies. This therapy is primarily being evaluated for the treatment of recurrent glioblastoma and other high-grade brain tumors, including pediatric high-grade glioma (pHGG) and diffuse intrinsic pontine glioma (DIPG).
