MSC-exos-P1 is an investigational exosome-based therapy derived from mesenchymal stem cells (MSCs) designed for delivery via nebulization. These extracellular vesicles contain a cargo of bioactive molecules, including proteins, lipids, and microRNAs, that exert immunomodulatory, anti-inflammatory, and anti-fibrotic effects. In the context of rapidly progressive interstitial lung disease (RP-ILD), MSC-exos-P1 is intended to modify alveolar macrophage phenotypes from a pro-inflammatory to an anti-inflammatory state, reduce the production of systemic pro-inflammatory cytokines, and suppress the activation of lung fibroblasts. The nebulized delivery system allows for direct targeting of the lung parenchyma, potentially mitigating the severe pulmonary inflammation and fibrosis associated with anti-MDA5 positive dermatomyositis.
