MSC-IL-2 N103R V106D is an experimental cell therapy consisting of mesenchymal stromal cells (MSCs) genetically modified to express a mutant version of murine interleukin-2 (IL-2). The specific mutations, N103R and V106D, are engineered to significantly reduce the cytokine's affinity for the IL-2 receptor beta (IL-2Rβ) while maintaining high affinity for the IL-2 receptor alpha (IL-2Rα/CD25). This bias results in the selective expansion and activation of regulatory T cells (Tregs), which constitutively express high levels of CD25, over pro-inflammatory effector T cells and NK cells. The therapy utilizes the innate ability of MSCs to home to sites of inflammation, where they locally secrete the mutant IL-2 to restore immune tolerance. It has been studied in preclinical models of autoimmune diseases, including rheumatoid arthritis and type 1 diabetes.
