MSC-Klotho is a cell therapy product consisting of mesenchymal stem cells (MSCs) genetically modified to express and secrete the anti-aging protein Klotho. Klotho is predominantly produced in the kidney and brain and has been shown to reduce inflammation, oxidative stress, fibrosis, and counter-regulate the renin-angiotensin-aldosterone system. Deficiency of Klotho is associated with aging-related diseases such as chronic kidney disease (CKD), acute kidney injury (AKI), heart failure following acute cardiac events, and neurodegenerative disorders including Alzheimer's disease. By engineering MSCs to overexpress Klotho, this therapy aims to deliver sustained levels of the protein directly to diseased tissues via systemic administration. Preclinical studies demonstrate that MSC-Klotho improves renal function more effectively than unmodified MSCs or extracellular vesicles from these cells in models of AKI. The mechanism involves both direct anti-inflammatory effects from the MSCs themselves and enhanced local therapeutic action from secreted Klotho protein[1][2][4][5][7].
