MSLN-CAR-IL7-IL7Rα NK cells, also known as the **Buddy-CAR system**, are engineered natural killer (NK) cells designed to treat mesothelin-expressing solid tumors, such as pancreatic cancer. Developed at the Dana-Farber Cancer Institute, these cells are modified with a chimeric antigen receptor (CAR) targeting mesothelin (MSLN) and are further engineered to both secrete interleukin-7 (IL7) and express the interleukin-7 receptor alpha (IL7Rα). This dual modification overcomes the natural lack of IL7Rα on mature NK cells, allowing for autocrine "cis-activation" that significantly enhances NK cell proliferation, survival, and cytotoxicity. Additionally, the secreted IL7 provides "trans-activation" to bystander tumor-infiltrating T cells within the immunosuppressive tumor microenvironment, promoting their proliferation and preventing exhaustion. Preclinical studies have demonstrated superior trafficking, proliferation, and anti-tumor responses in xenograft mouse models compared to standard CAR-NK constructs.
