Overview

MSLN CAR NK cells are an experimental cell-based immunotherapy consisting of natural killer (NK) cells engineered to express a chimeric antigen receptor (CAR) specific for mesothelin (MSLN). Mesothelin is a cell surface glycoprotein overexpressed in various solid tumors—including pancreatic ductal adenocarcinoma, ovarian cancer, and mesothelioma—as well as in specific subtypes of acute myeloid leukemia (AML), such as KMT2A-rearranged AML. The therapy utilizes the innate cytotoxic potential of NK cells, which are redirected by the CAR (often incorporating 4-1BB and CD3ζ signaling domains) to recognize and lyse MSLN-expressing cells. Compared to CAR-T cell therapies, CAR-NK cells are being explored for their potentially superior safety profile, including a lower risk of cytokine release syndrome and the potential for off-the-shelf allogeneic use.
