MT-8421 is a novel engineered toxin body (ETB) designed to target and deplete cells expressing cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), particularly regulatory T cells (Tregs) within the tumor microenvironment. The drug consists of biparatopic heavy chain variable domains (VHHs) that bind to human CTLA-4 and deliver a ribosome-inactivating payload, leading to selective destruction of CTLA4+ cells via inhibition of protein synthesis. This mechanism aims to dismantle the immunosuppressive tumor microenvironment by eliminating Tregs, thereby enhancing anti-tumor immune responses. MT-8421 is being developed by Molecular Templates for use in advanced solid tumors where CTLA-4 inhibitors have shown benefit or where tumors are known for an immune-rich microenvironment. It is currently under investigation as both monotherapy and in combination with PD-1 inhibitors such as nivolumab[2][5][7][9].
