MTbLF

MTbLF (Metrodora Therapeutics bovine Lactoferrin) is an investigational biologic therapeutic developed by Metrodora Therapeutics for the treatment and prevention of recurrence of bacterial vaginosis (BV). The drug's active ingredient is bovine lactoferrin (bLF), an 80 kDa iron-binding glycoprotein of the transferrin family derived from bovine milk. Formulated as a bilayer vaginal tablet containing 300 mg of bLF, MTbLF is designed for intravaginal administration. Its primary mechanism of action involves sequestering iron within the vaginal environment, thereby depriving iron-dependent pathogenic bacteria, such as *Gardnerella vaginalis*, of a critical nutrient while sparing iron-independent commensal lactobacilli. Additionally, the drug exerts direct antimicrobial effects through lactoferricin, a polycationic peptide sequence within the protein that binds to and disrupts microbial cell surfaces. MTbLF has been evaluated in Phase 1 clinical trials (e.g., ACTRN12619000295145) to assess its safety, tolerability, and vaginal pharmacokinetics in both healthy women and those with BV.
