MTX-ATMPs (methotrexate-loaded autologous tumor cell-derived microparticles) is an experimental autologous biologic therapy developed by Union Hospital, Tongji Medical College, Huazhong University of Science and Technology for the treatment of malignant pleural effusion (MPE), particularly in patients with advanced non-small cell lung cancer. This individualized drug delivery platform utilizes microparticles derived from the patient's own tumor cells to encapsulate methotrexate, facilitating targeted delivery to the pleural space via intrapleural infusion. The therapy exerts its effect through dual mechanisms: direct cytotoxicity mediated by methotrexate (a dihydrofolate reductase inhibitor) and the reprogramming of the immunosuppressive tumor microenvironment. It has been shown to reduce CD163+ M2-like macrophages and stimulate CD4+ and CD8+ T cells to release pro-inflammatory cytokines such as IL-2 and IFN-gamma, thereby enhancing the local anti-tumor immune response.
