Mulberry total alkaloid tablet is a botanical drug extracted from the twigs of *Morus alba* (mulberry). Developed by Beijing Wehand-Bio Pharmaceutical in collaboration with the Institute of Materia Medica, Chinese Academy of Medical Sciences, it was approved by the NMPA in China in 2020 for the treatment of type 2 diabetes mellitus. The drug consists of a group of polyhydroxy alkaloids, with 1-deoxynojirimycin (DNJ), fagomine, and 1,4-dideoxy-1,4-imino-D-arabinitol (DAB) as the primary active constituents. It functions as a potent alpha-glucosidase inhibitor, effectively delaying the digestion of carbohydrates and the subsequent absorption of glucose in the small intestine, thereby lowering postprandial blood sugar levels. In addition to its use in diabetes, it is being evaluated in Phase 4 clinical trials for polycystic ovary syndrome (PCOS) to address metabolic dysfunction and insulin resistance.
