Muraglitazar + metformin is an oral combination therapy investigated for the treatment of type 2 diabetes mellitus. Muraglitazar is a small molecule dual peroxisome proliferator-activated receptor (PPAR) agonist, activating both PPAR alpha and gamma subtypes, which improves glycemic control and modulates lipid profiles by reducing insulin resistance, lowering triglycerides, and increasing HDL cholesterol. Metformin, a well-established biguanide antihyperglycemic, acts primarily by reducing hepatic glucose production and improving insulin sensitivity. The combination has been studied in multiple randomized, double-blind clinical trials and was shown to produce greater reductions in hemoglobin A1c, triglycerides, and non-HDL cholesterol, with greater increases in HDL cholesterol compared to pioglitazone plus metformin or metformin monotherapy in patients with type 2 diabetes inadequately controlled with metformin alone. However, the combination was associated with increased risks of edema, weight gain, and higher rates of cardiovascular and heart failure events than comparators. Development of muraglitazar was discontinued due to safety concerns regarding cardiovascular outcomes[1][2][3][4][5].
