The mutant-CALR peptide vaccine is an investigational cancer immunotherapy developed by Dr. Marina Kremyanskaya at the Icahn School of Medicine at Mount Sinai. It is designed to treat myeloproliferative neoplasms (MPNs), specifically myelofibrosis and essential thrombocythemia, in patients harboring calreticulin (CALR) exon 9 mutations. The vaccine consists of synthetic peptides corresponding to the neoantigenic C-terminus of the mutant CALR protein, which is created by frameshift mutations and is shared across approximately 30% of MPN patients. It is administered alongside the adjuvant Poly-ICLC (a TLR3 agonist) to enhance the induction of CALR-specific cytotoxic and helper T-cell responses, aiming to selectively recognize and eliminate malignant hematopoietic cells expressing the mutant protein.
