The mutant-calreticulin peptide vaccine is a therapeutic cancer vaccine targeting the neoantigenic C-terminal tail of the calreticulin (CALR) protein. In many patients with myeloproliferative neoplasms (MPNs), specifically essential thrombocythemia and myelofibrosis, frameshift mutations in exon 9 of the CALR gene result in a novel, shared amino acid sequence at the C-terminus that is absent in normal cells. This vaccine utilizes synthetic long peptides corresponding to this mutant C-terminus to stimulate the patient's immune system. Its mechanism of action involves the induction of specific CD4+ and CD8+ T-cell responses that recognize and eliminate malignant hematopoietic cells expressing the mutant CALR protein. This approach aims to provide a targeted immunotherapy for MPN patients who harbor these specific mutations.
