Overview

Mutant pro-urokinase is a genetically engineered variant of prourokinase (proUK), a zymogenic plasminogen activator. The most studied form, known as M5 (Lys300→His mutation), was developed to improve plasma stability and reduce nonspecific activation to urokinase, thereby enhancing fibrin specificity and safety. Mutant pro-urokinase acts by converting plasminogen to plasmin primarily at the site of fibrin clots, promoting targeted thrombolysis while minimizing systemic bleeding risks. It is designed for use in acute thrombotic conditions such as ischemic stroke and myocardial infarction, often in combination with low-dose alteplase for sequential or dual thrombolytic therapy. Clinical studies have shown that it can achieve effective clot lysis with reduced risk of intracranial hemorrhage compared to standard therapies[1][3][6][7]. Thrombolytic Science LLC has been developing this agent under the name HisproUK[5].
