Overview

Muzastotug (ADG126) is a novel anti-CTLA-4 IgG1 monoclonal antibody developed using SAFEbody® technology. This technology incorporates cleavable masking peptides that keep the antibody inactive in circulation and preferentially activate it within the tumor microenvironment. Muzastotug binds to a unique epitope on CTLA-4, priming T cells and depleting regulatory T cells (Tregs), thereby enhancing antitumor immune responses. Unlike traditional CTLA-4 inhibitors, its design aims to reduce systemic toxicity while maintaining or improving efficacy. Muzastotug has demonstrated promising clinical activity in combination with pembrolizumab (an anti-PD-1 therapy) for microsatellite stable colorectal cancer (MSS CRC), particularly in patients without liver metastases. Clinical studies have shown meaningful response rates and a favorable safety profile compared to existing CTLA-4 inhibitors[6][8][10].
