Overview

Mycobacterium vaccae is a non-pathogenic, saprophytic environmental bacterium used as an immunotherapeutic agent. It is typically administered as a whole-cell, heat-killed vaccine or oral tablet. The drug acts primarily by modulating immune responses—specifically, it induces regulatory T cells and shifts T-helper cell activity to reduce inappropriate Th2 responses and promote Th1-type immunity. This immunomodulatory effect has been investigated for adjunctive treatment of tuberculosis (including multidrug-resistant TB), atopic dermatitis, asthma, and other inflammatory conditions. Its anti-inflammatory properties are also being explored for stress resilience and prevention of allergic airway inflammation[1][3][4][5][6]. Mycobacterium vaccae-based products have received orphan drug status in the US for tuberculosis[8].
