This combination therapy regimen consists of mycophenolate mofetil (MMF) and a corticosteroid, specifically investigated for the treatment of advanced IgA nephropathy. Mycophenolate mofetil is a small molecule immunosuppressant that acts as a potent, selective, uncompetitive, and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH), which is the rate-limiting enzyme in the de novo synthesis of guanosine nucleotides; this inhibition primarily affects T and B lymphocytes which rely on this pathway for proliferation. Corticosteroids are a class of steroid hormones that bind to the glucocorticoid receptor to exert broad anti-inflammatory and immunosuppressive effects by modulating gene transcription. This specific regimen was evaluated in a Phase IV clinical trial (NCT02353910) led by Yonsei University and sponsored by ChongKunDang Pharmaceutical, focusing on patients with advanced disease (eGFR 20-50 mL/min/1.73 m²) to reduce proteinuria and preserve renal function.
