Mycophenolic acid (MPA), also known as mycophenolate acid, is an immunosuppressant drug primarily used to prevent rejection in patients receiving solid organ transplants such as kidney, heart, or liver. It acts by inhibiting inosine monophosphate dehydrogenase (IMPDH), a key enzyme in the de novo synthesis of guanine nucleotides required for lymphocyte proliferation. By blocking this pathway, MPA suppresses the immune response and reduces the risk of transplant rejection. Mycophenolic acid is also sometimes used off-label for autoimmune diseases and inflammatory conditions[2][4]. The compound itself was historically administered directly but is now more commonly delivered via prodrugs such as mycophenolate mofetil or mycophenolate sodium to improve bioavailability and reduce gastrointestinal side effects[7][9].
