Mycophenolic acid-pretreated mesenchymal stem cells (MPA-MSCs) are an experimental cell therapy product consisting of mesenchymal stem cells (MSCs) that have been ex vivo conditioned with mycophenolic acid (MPA) prior to administration. MPA is a potent immunosuppressant that works by inhibiting **inosine monophosphate dehydrogenase (IMPDH)**, an enzyme essential for de novo purine synthesis. When MSCs are exposed to MPA during culture, they undergo a priming effect that significantly enhances their immunomodulatory and anti-inflammatory properties. This pretreatment typically leads to the upregulated expression of key immunosuppressive mediators, such as **indoleamine 2,3-dioxygenase (IDO)** and **prostaglandin E2 (PGE2)**. These modified cells are being investigated for their ability to more effectively suppress T-cell proliferation and promote the induction of regulatory T-cells (Tregs), offering a potentially superior therapeutic option for treating severe immune-mediated conditions like graft-versus-host disease (GvHD) and systemic lupus erythematosus (SLE) compared to naive MSCs.
