Overview

Mydriasert is a hospital-only ophthalmic insert used to achieve pre-operative mydriasis (pupil dilation) or for diagnostic purposes when monotherapy is insufficient. The insert contains two active ingredients, phenylephrine (an alpha sympathomimetic) and tropicamide (an anticholinergic), which are gradually released into the eye after placement in the lower conjunctival fornix. Phenylephrine stimulates the iris dilator muscle via alpha-adrenergic receptor agonism, while tropicamide blocks muscarinic acetylcholine receptors on the pupillary sphincter muscle, leading to relaxation and further pupil dilation. This dual mechanism provides a wide and stable mydriasis suitable for surgical procedures such as cataract surgery. Mydriasert offers advantages over traditional eye drops by improving local efficacy with limited systemic absorption and enhanced patient comfort[1][3][5][7].
