myo-Inositol trispyrophosphate (ITPP) is a synthetic small molecule derived from phytic acid, classified as an inositol phosphate and pyrophosphate. It acts as a membrane-permeant allosteric effector of hemoglobin, reducing hemoglobin's oxygen-binding affinity and thereby shifting the oxygen–hemoglobin dissociation curve to the right. This facilitates increased release of oxygen from red blood cells into tissues, improving tissue oxygenation[1][3][5]. ITPP has been investigated for its potential to enhance physical endurance and performance due to these effects, leading to its inclusion on anti-doping lists[1][3][8]. Preclinical studies have shown benefits in models of heart failure and cancer by alleviating tissue hypoxia; it also stabilizes tumor vasculature via PTEN activation[5]. Clinical trials have explored its use as an adjuvant therapy for cancer (notably under the code name OXY111A), chronic heart failure, obesity, infertility, polycystic ovary syndrome (PCOS), and other conditions associated with hypoxia or vascular dysfunction[4].
