Overview

N-104 is a synthetic 15-amino acid peptide derived from the C-terminus of lacritin, a human plasma glycoprotein. Developed by IsletRegen, LLC, in collaboration with researchers from the University of Virginia and the University of Miami, N-104 is designed as a dual-action immunoregulatory and regenerative therapy for the treatment of early-onset type 1 diabetes (T1D). The peptide functions by directly promoting the proliferation of pancreatic β-cells and protecting them from inflammatory stress by suppressing cytokine-induced HLA-I upregulation. In preclinical models, including human pancreatic slices and NOD mice, N-104 has demonstrated the ability to restore normoglycemia, reduce insulitis, and suppress pathogenic T- and B-cell populations. A PEGylated version, PEG-N-104, has been developed to enhance the drug's pharmacokinetic properties and has shown long-term efficacy in halting diabetes progression.
