The combination of n-acetyl cysteine (NAC), alpha-lipoic acid (ALA), and liposomal glutathione is an antioxidant-based therapeutic regimen developed by Medical Research Laboratories International (MRLI). This formulation was primarily investigated in Phase 2 clinical trials for the treatment of hypertriglyceridemia, specifically as an adjunctive therapy to manage the hepatic side effects associated with microsomal triglyceride transfer protein (MTP) inhibitors like implitapide. MTP inhibitors are effective at lowering triglycerides but often cause hepatic steatosis (fatty liver) and elevated transaminases due to the accumulation of lipids in hepatocytes. NAC serves as a precursor to the endogenous antioxidant glutathione, ALA acts as a versatile fat- and water-soluble antioxidant and mitochondrial enzyme cofactor, and the liposomal delivery of glutathione provides a direct source of the tripeptide with enhanced bioavailability. Together, these components aim to reduce oxidative stress, support hepatic detoxification, and improve the safety profile of primary lipid-lowering agents.
