**N-methyl-L-arginine hydrochloride (546C88)** is a nitric oxide synthase inhibitor developed for investigation in patients with septic shock. Its mechanism of action involves inhibition of nitric oxide synthase, moderating excessive production of nitric oxide that contributes to vasodilation in septic shock. In a clinical study, intravenous infusion of 546C88 (at 1–20 mg/kg/hr for up to 8 hours) increased vascular tone and reduced norepinephrine requirements while maintaining satisfactory blood pressure without major adverse effects. The drug was developed by Glaxo (now part of GSK), primarily targeted at restoring vasomotor tone and stabilizing hemodynamics in septic shock patients[1].
