nab-sirolimus + adagrasib

nab-sirolimus + adagrasib is an investigational combination therapy consisting of nab-sirolimus (Fyarro), an intravenous nanoparticle albumin-bound formulation of the mTOR inhibitor sirolimus, and adagrasib (Krazati), an oral small-molecule inhibitor of mutant KRAS G12C. Developed through a clinical collaboration between Aadi Bioscience and Mirati Therapeutics (now a subsidiary of Bristol Myers Squibb), this combination is designed to simultaneously inhibit the mTOR and KRAS G12C signaling pathways. Preclinical studies demonstrated that dual inhibition of these pathways addresses key bypass and feedback resistance mechanisms, leading to enhanced anti-tumor activity compared to either agent alone. The combination was evaluated in the Phase 1/2 KRYSTAL-19 trial (NCT05840510) for patients with advanced solid tumors and non-small cell lung cancer (NSCLC) harboring a KRAS G12C mutation; however, the trial was terminated in 2024 due to changes in business objectives.
