nab-sirolimus + letrozole is an investigational combination of two drugs: nab-sirolimus, a nanoparticle albumin-bound intravenous mTOR inhibitor, and letrozole, an orally administered aromatase inhibitor. This combination is under phase 2 clinical investigation for the treatment of advanced or recurrent endometrioid endometrial carcinoma (EEC), especially in patients with 0–1 prior lines of chemotherapy in the recurrent/advanced setting. nab-sirolimus targets dysregulated mTOR signaling, which is frequently implicated in EEC, and letrozole inhibits estrogen receptor signaling by depleting estrogen levels. Preclinical evidence and prior clinical trials with similar agents suggest this dual inhibition may address resistance to hormonal therapy and provide synergistic antitumor effects in EEC, where both the PI3K/AKT/mTOR and estrogen receptor pathways are commonly altered. nab-sirolimus is already approved (as a single agent) in the US for malignant perivascular epithelioid cell tumor (PEComa), but this specific combination is experimental and not approved for any indication[1][2][3][4][5].
