Nadofaragene firadenovec (Adstiladrin) is a non-replicating adenoviral vector-based gene therapy developed for the treatment of adult patients with high-risk, Bacillus Calmette-Guérin (BCG)-unresponsive non-muscle invasive bladder cancer (NMIBC). The therapy utilizes a recombinant adenovirus type 5 (Ad5) vector to deliver the human interferon alpha-2b (IFNα2b) gene directly into the bladder epithelium. It is formulated with Syn3, a polyamide surfactant excipient that facilitates the transduction of the viral vector into the urothelial cells. Once inside, the cells act as "microfactories," producing and secreting high local concentrations of the IFNα2b protein. This localized production induces a potent antitumor immune response and inhibits cancer cell proliferation while minimizing the systemic toxicity typically associated with systemic interferon administration.
