Nadroparin calcium + warfarin is a sequential combination anticoagulation regimen used primarily for the treatment and prevention of thrombotic events, such as portal vein thrombosis (PVT) in patients with cirrhosis. Nadroparin calcium is a low molecular weight heparin (LMWH) that acts by binding to antithrombin III and accelerating the inactivation of factor Xa and, to a lesser extent, factor IIa (thrombin), thereby inhibiting clot formation. Warfarin is an oral vitamin K antagonist that inhibits the synthesis of vitamin K-dependent clotting factors II, VII, IX, and X by blocking vitamin K epoxide reductase complex subunit 1 (VKORC1), leading to reduced blood coagulation. The sequential use typically involves initial short-term administration of nadroparin calcium via subcutaneous injection followed by longer-term oral warfarin therapy. This approach has been shown to be effective and safe for recanalization of PVT in cirrhotic patients without significantly increasing bleeding risk[1][2][3][4].
