This refers to a combination therapy protocol developed and studied at Shaare Zedek Medical Center for luteal phase support in women undergoing natural cycle frozen embryo transfer (FET) for infertility. The protocol typically involves the administration of a Gonadotropin-Releasing Hormone (GnRH) agonist, such as nafarelin (Synarel) or triptorelin (Decapeptyl), in conjunction with human chorionic gonadotropin (hCG). The mechanism of action involves the GnRH agonist stimulating the pituitary gland to secrete luteinizing hormone (LH) and potentially exerting direct beneficial effects on the endometrium and the developing embryo via GnRH receptors. The addition of hCG further supports the corpus luteum's function. This combination is compared against standard vaginal progesterone to improve pregnancy rates in natural cycle FET.
