Nafcillin is a semi-synthetic, beta-lactamase-resistant penicillin antibiotic used primarily to treat infections caused by susceptible staphylococcal bacteria, especially those resistant to other penicillins but not methicillin-resistant Staphylococcus aureus (MRSA)[2][4][5]. It acts by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), thereby blocking the final transpeptidation step of peptidoglycan synthesis and leading to cell lysis and death[2][6][8]. Nafcillin is indicated for bacteremia, skin and soft tissue infections, respiratory tract infections, bone and joint infections, urinary tract infections (UTIs), and endocarditis caused by susceptible organisms[1][4][5]. It is administered intravenously or intramuscularly. Nafcillin was developed as a narrow-spectrum antistaphylococcal agent with high resistance to inactivation by staphylococcal penicillinase[2].
