Nafurafine is a highly selective kappa opioid receptor (KOR) agonist originally developed by Toray Industries. It is primarily utilized for the treatment of resistant pruritus, particularly uremic pruritus in patients undergoing hemodialysis and cholestatic pruritus in patients with chronic liver disease. Unlike mu-opioid receptor agonists, nafurafine does not typically produce significant sedative effects, respiratory depression, or physical dependence. Its mechanism of action involves the activation of kappa receptors, which are thought to modulate and inhibit the transmission of itch signals that are often exacerbated by mu-opioid system overactivity. Beyond its established use in pruritus, nafurafine is being investigated for its potential to treat protein-energy wasting (PEW) in uremic patients, where it may help improve appetite and nutritional status.
