Naive T cell depleted donor lymphocyte infusion is a form of adoptive immunotherapy in which donor lymphocytes (white blood cells) are collected from the original hematopoietic stem cell transplant donor and infused into the recipient after transplantation. In this modified version, the product is specifically processed to deplete CD45RA+ naive T cells before infusion. The rationale for depleting naive T cells is based on evidence that these cells are primary drivers of graft-versus-host disease (GvHD), a serious complication of allogeneic transplants. By removing them, the risk of acute and chronic GvHD may be reduced while preserving anti-tumor (graft-versus-leukemia or graft-versus-tumor) effects mediated by memory and effector T cells. This approach has been studied primarily as prophylactic or preemptive therapy to enhance immune recovery and reduce relapse rates in patients with hematologic malignancies following allogeneic hematopoietic stem cell transplantation[1][2]. The mechanism involves adoptive transfer of selected immune effector cells to mediate anti-cancer activity with minimized toxicity.
