Nal-OX40 is an investigational combination immunotherapy regimen comprising the opioid antagonist naltrexone and an OX40 (CD134) agonist monoclonal antibody, most notably MEDI6469. This therapeutic strategy leverages the immune-modulatory properties of low-dose naltrexone (LDN), which is hypothesized to enhance immune surveillance by transiently blocking opioid receptors, leading to a compensatory increase in endogenous opioids and their receptors on immune cells. When combined with an OX40 agonist, which provides a potent co-stimulatory signal to effector T cells and helps suppress regulatory T cell activity, the regimen aims to synergistically stimulate a robust anti-tumor immune response. Nal-OX40 is primarily being evaluated for the treatment of advanced solid tumors that have proven resistant to standard checkpoint inhibitors and other therapies.
