Overview

Nalbuphine + remifentanil is a combination of two opioid analgesics used primarily for pain management in surgical and intensive care settings. Nalbuphine is a semi-synthetic opioid that acts as a kappa opioid receptor agonist and mu opioid receptor antagonist, providing moderate to strong analgesia with reduced risk of respiratory depression compared to pure mu agonists. Remifentanil is an ultra-short-acting synthetic opioid that acts as a potent mu opioid receptor agonist, characterized by rapid onset and offset due to its metabolism by nonspecific blood and tissue esterases. The combination may be used for balanced anesthesia or procedural sedation, leveraging the rapid titratability of remifentanil with the ceiling effect on respiratory depression provided by nalbuphine[4][6]. Both drugs are administered intravenously.
