**Naltrexone + NAD+** is a combination therapy consisting of low-dose naltrexone (typically 4.5 mg/day) and supplementation with NAD+ (nicotinamide adenine dinucleotide), often delivered via iontophoresis patches or intravenous infusions. Naltrexone is a mu-opioid receptor antagonist traditionally approved for opioid and alcohol dependence, but at low doses, it acts as a glial modulator inhibiting microglial activation and Toll-like receptor 4, leading to anti-inflammatory and immune-modulating effects. NAD+ is a coenzyme central to cellular energy production, mitochondrial function, DNA repair, and healthy aging. The combination is being studied as a potential treatment for persistent fatigue and impaired quality of life in long COVID/post-acute sequelae of SARS-CoV-2 infection. Pilot studies report improved fatigue scores and overall quality of life in about half of treated patients, with a generally mild safety profile[1][2][3][4][6].
