Nandrolone is a classic anabolic-androgenic steroid (AAS) and a 19-nor derivative of testosterone. It is primarily administered as long-acting esters, most notably nandrolone decanoate, via intramuscular injection. Clinically, nandrolone is valued for its high ratio of anabolic to androgenic activity, allowing it to promote muscle growth, increase bone mineral density, and stimulate erythropoiesis with significantly fewer virilizing side effects compared to testosterone. Its primary mechanism involves binding to and activating the androgen receptor, which modulates gene expression related to protein synthesis and nitrogen retention. It has been used historically for the treatment of various anemias, osteoporosis in postmenopausal women, and breast cancer. Modern clinical investigation focuses on its utility in managing sarcopenia, cancer-associated cachexia, HIV-associated wasting syndrome, and recovery from severe burns or major orthopedic surgeries.
