Nangibotide is a synthetic peptide and the first clinical-stage drug candidate targeting the triggering receptor expressed on myeloid cells 1 (TREM‑1), an immunoreceptor found on neutrophils, monocytes, macrophages, natural killer cells, and endothelial cells. TREM‑1 amplifies inflammatory responses following activation by its ligand. Nangibotide acts as a decoy receptor or inhibitor of TREM‑1 by binding to its ligand and preventing activation of the receptor. This mechanism blocks downstream signaling pathways such as NF‑κB activation, thereby reducing production of pro-inflammatory mediators and modulating innate immune responses without impairing host defense against infection. The drug is being developed primarily for acute inflammatory disorders such as septic shock; it has demonstrated safety in Phase 1 studies with healthy volunteers and efficacy signals in Phase 2 trials with patients suffering from septic shock[1][3][5][7].
