Nano-calcium hydroxide paste is an investigational nanoparticle-based formulation of calcium hydroxide (Ca(OH)₂) intended for use as an intracanal medicament in endodontic procedures. By utilizing nano-sized particles, the formulation aims to achieve superior penetration into dentinal tubules and enhanced antimicrobial efficacy compared to conventional calcium hydroxide pastes. It is primarily being studied for its ability to modulate periapical inflammation and eliminate persistent microorganisms, such as *Enterococcus faecalis*, in patients undergoing endodontic retreatment for apical periodontitis. The high surface-area-to-volume ratio of the nanoparticles increases chemical reactivity and the release of hydroxyl ions, creating an alkaline environment lethal to bacteria. The drug is currently being evaluated in academic clinical trials, notably by Al-Azhar University.
