Nano-insulin gel is an experimental topical formulation of insulin developed by researchers at Deraya University for the management of musculoskeletal inflammation, specifically shoulder joint inflammation and osteoarthritis. The formulation is designed to be used as a coupling medium in phonophoresis-assisted physiotherapy, leveraging nanotechnology to enhance the transdermal delivery of insulin to localized joint tissues. Its mechanism of action involves the activation of the insulin receptor and the subsequent PI3K/Akt signaling pathway in synovial fibroblasts. This activation leads to the downregulation of cartilage-degrading enzymes, including matrix metalloproteinases (MMP-1, MMP-13) and ADAMTS4, thereby providing localized anti-inflammatory and chondroprotective effects while minimizing systemic exposure.
